Please complete the forms and submit to the administrative office in the school of nursing (B637)

(I) Degree Thesis Examination Eligibility Application

• To qualify as a doctoral candidate in this department, one must complete required and elective courses, participate in international learning activities, and fulfill the academic achievement assessment and academic ethics education requirements. Proof of course completion must be submitted with the relevant application documents to the department's Master's and Doctoral Review Committee by February 27, April 30, August 31, or October 31 each year.

(II) For Degree Thesis Examination, complete the relevant forms and submit them to the office handler. Instructions for using the forms are as follows:

1. One month before the oral exam, submit:

 2. On the day of the oral exam, provide:

3. For cancellation of the degree examination, use:
