Please carefully read the Graduate Institute’s Master's Degree Examination Regulations before applying for the degree examination. After completing the relevant forms, submit them to administrative office in school of nursing (B637). The instructions for using the forms are as follows:

1. One month before the examination, submit the following forms:

o Annual transcripts (歷年成績單) o The first six chapters of the thesis (論文前6章).

o Academic Research Ethics Education Certificate (學術研究倫理教育課程AREE證明)

o B12-F048-01-en -Academic Achievement Record for Graduate Students 碩士應屆畢業生成績審核表

o B12-F063-01-en -Form for Reviewing the Professional Consistency of Dissertations (學位論文主題專業領域相符審核表)

o B12-F079-01-en--Graduate Student Dissertation Originality Check Results Form(研究生學位論文原創性比對檢核表) & first and last pages of the originality check report

o B12-F045-01-en -Application for Master's Degree Examination (碩士學位考試申請書)

o Hospital IRB Approval Certificate (醫院IRB通過證明)

2. On the day of the examination, provide:

o B12-F046-01-en -Master's Degree Examination Score Sheet 碩士學位考試成績表

o 8-2-4 Comments Sheet of Committee in Oral Defense for Master degree(論文學位考試口試審查意見表)

o B12-F047-01-en -Master's Degree Examination Approval Form ( 碩士學位考試委員會審定書

o Receipt for Committee Members (領款收據)

o Application form of payment for Committee Members(口試費用申請表)

3. Before submitting your Master’s thesis, make sure to thoroughly read the “Master's Thesis Writing Format Explanation and Examples” (「碩士論文撰寫格式說明與範例」) to ensure compliance with the format requirements.

o The first and last pages of the originality check report should be attached in the dissertation's appendix
