Instructions for the PhD proposal exam:

Please complete the forms and submit to the administrative office in the school of nursing (B637)

1. One month before the proposal oral exam, submit:

o 9-2-1博士班論文計畫審查申請表Application form for Dissertation Proposal Review(before proposal)

o 9-2-2博士論文計畫口試審查申請表Application form for Proposal Defense(before proposal)

o B12-F063-01-en-學位論文主題專業領域相符審核表-Form for Reviewing the Professional Consistency of Dissertations(before proposal)

2. On the day of the proposal oral exam, provide:

o 9-2-3 博士論文計畫口試結果 Results of PhD dissertation proposal defense (on day of proposal)

o 9-2-4 博士論文計畫口試審查意見表 Results & Comments for Dissertation Proposal (on day of proposal)

o Receipt for Committee Members (領款收據)

o Application form of payment for Committee Members(口試費用申請表)

Note: Form 9-2-2 and the B12-F063-01 must be approved by the Master's and Doctoral Review Committee. The committee meets twice each semester for reviews; in the first semester at the beginning of September and November, and in the second semester at the beginning of March and May. The deadlines for submission of applications are August 31 and October 31 for the first semester, and February 27 and April 30 for the second semester.
